A referral is required from the individual's primary physician in order to complete an occupational therapy evaluation.
Once a referral is received, an intake packet will be sent to the client and/or caregivers to be filled out. After the intake packet is completed, an occupational therapy evaluation can be scheduled. The initial evaluation will determine the strengths and weaknesses of the client and what goals will be addressed during treatment sessions.

Treatment sessions will typically be scheduled once a week for 1-hour. The client and/or caregivers will be notified if there is a change in the schedule.
